The U.S. Military medical worked like that when I had use of it.
There was paperwork when they wanted to admit me but I was married to a DODs civilian at the time and not totally military. Otherwise it was first in, first treated for normal health care.
I expect that you could still purchase supplementary health insurance.

I have health insurance from a major carrier but the doctor I had decided he wasn't being reimbursed enough so he's no longer in my PPO. He's nice and gives me discounts for my health care but from a $20 co-pay the bill then is more than $100 for annual physicals. He knows me, he's treated my mother, so he knows the family history which is wierd in his words but this just isn't doing it.
